At the title screen, press A, B, Right, A, C, A, Down, B, Right, A. After you do this, press and hold A + B + C and press Start. While playing you can use the following codes to change levels.
Forward level: A + UP
Forward stage: A + RIGHT
Back level: A + DOWN
Back stage: A + LEFT
